In Liberty City, Niko Bellic, an Eastern European,[23] arrives to meet his cousin Roman. Niko comes to Liberty City to pursue the American Dream, and to search for the man who betrayed his unit in a war fifteen years prior. After his arrival, however, Niko quickly learns that Roman's tales of riches and luxury were lies concealing struggles with gambling debts and loansharks, and that Roman lives in a dirty apartment rather than a mansion.
Niko defends Roman from his loansharks several times, eventually killing Vlad Glebov, Roman's Russian loanshark. Niko was forced to work for Vlad due to Roman's debts but ended up killing him after learning that that he had slept with Roman's girlfriend, Mallorie. After Vlad's murder, Niko and Roman are kidnapped by members of the Liberty CityBratva on order of Mikhail Faustin and his associate, Dimitri Rascalov. Faustin, not bothered by the murder of Vlad, hires Niko. Niko quickly learns that Faustin is a psychopath when he orders him to kill the son of Kenny Petrović, the most powerful man in the Liberty City Bratva. After the Petrović family threatens retaliation, Dimitri orders Niko to assassinate Faustin. When Niko meets with Dimitri to collect on the assassination, Dimitri betrays him. When Niko meets Dimitri, the latter brings his friend and Niko's former employer Ray Bulgarin, who accuses Niko of stealing from him. Niko denies the allegation saying that a cargo ship that he was transporting for Bulgarin across the Adriatic Seasank and that he nearly drowned while swimming to shore.
Immediately afterwards, Niko and Roman are forced to escape to Bohan when their apartment and taxi company are destroyed in arson attacks by Dimitri's men. However, things go poorly in Bohan: Dimitri's men kidnap Roman in a failed bid to ambush and kill Niko, who rescues Roman. Furthermore, one of the drug deals that Niko works on turns out to be a sting and another is busted. After the latter incident, Niko's current girlfriend Michelle then reveals that she works for a government agency and entraps Niko into working for her agency, known only by its cover: United Liberty Paper. Niko kills several known or suspected terrorists for the agency in exchange for the file of the numerous crimes the police have on him and the promise of assistance in finding the traitor of his unit.

The story begins with Jonathan "Johnny" Klebitz and all the members of the famous biker gang, The Lost Motorcycle Club, driving through the streets of Alderney at night. In a brief cameo, Niko Bellic, the main character of Grand Theft Auto IV, swears at a passer-by on the street as the gang drives past.
Johnny is the vice president of the gang, but has been the acting president for a year because the club's president, Billy Grey, has been in court-ordered rehab after being arrested for narcotics possession. As leader, Johnny has established The Lost throughout Liberty City by making truces and deals after suffering several financial troubles, mainly caused by Billy's poor leadership. The most notable truce is with The Lost's rivals, The Angels of Death, which has allowed The Lost to operate their drug deals and gun running operations more freely.
On being released from rehab, Billy immediately reverts to a life of crime and tensions quickly grow between Billy and Johnny, who believe the gang should go different ways. Johnny wants to keep the gang running smoothly, deeming war with other gangs as immature and bad for business, whereas Billy prefers acts of random violence and mayhem. As such, Billy quickly leads the Lost to assault the Angels of Death Clubhouse, beginning a gang war against the Angels of Death, which serves as the main backdrop for the game.
Meanwhile Johnny does business with drug dealer Elizabeta Torres and import bikers Malc and DeSean. He is also entrapped by Congressman Thomas Stubbs III into committing several assassinations and planting a bug in the car of Deputy Mayor Bryce Dawkins (which is given to Niko Bellic by Bernie Crane soon after in Grand Theft Auto IV).
From time to time Johnny is led to help his ex-girlfriend Ashley, a drug addict who constantly gets herself into trouble, including saving her from a group of thugs and paying off her debts to Dimitri Rascalov by kidnapping Roman Bellic.
Eventually, Billy is arrested after a failed drug deal with the Triads (which was actually partly set up by Billy, as revealed in the Ballad of Gay Tony) and Johnny takes over as President of The Lost. This immediately sparks a civil war within the gang, one side with Johnny, the other with secretary Brian Jeremy, who is still faithful to Billy and believes Johnny to be responsible for Billy's arrest. Brian stages a coup against Johnny in which most of the Lost are killed. Johnny and his friends manage to come out on top, but Brian escapes. Soon after mafia capo Ray Boccino sells Brian's location to Johnny in the interest of stabilizing his business with the Lost. Johnny goes to the safehouse and kills Brian along with the last of his goons, putting an end to the civil war.
After killing Brian, Johnny repays Boccino by stealing $2 million worth of diamonds for him from Anthony "Gay Tony" Prince (while killing Tony's boyfriend Evan Moss in the process). When Johnny tries to collect his share of the diamond money, he becomes part of the infamous diamond deal seen in Grand Theft Auto IV, and manages to get away with all of the $2 million that Boccino was to receive for the diamonds. Johnny gives the money to the Lost's treasurer Jim Fitzgerald. Soon after both Jim and Johnny are held captive by Boccino, but fight their way free and are pursued by Boccino's men. Johnny leads his pursuers into an ambush in Alderney. However, when he returns to the Lost Clubhouse, Johnny learns from Ashley that Jim has been killed (by Niko Bellic).
Shortly after, Stubbs reveals that Billy is planning to testify against the Lost, pin all its crimes on Johnny, and enter Witness Protection Program. Johnny leads the remaining members of the Lost in a daring raid on the state prison in Alderney, where Billy is being detained, and personally executes him. With Billy dead, the surviving members of the Lost return to their clubhouse, where it is revealed that the only surviving members are Johnny, Clay Simons, Terry Thorpe and the paraplegic Angus Martin. With their club and brotherhood destroyed, they decide to burn the clubhouse to the ground. The last shot is of them watching it burn, contemplating what comes next. Ashley promises Johnny that she will enter rehab, with Johnny wishing her the best.
At several points throughout the course of the game, the plot intertwines with events from Grand Theft Auto IV and is seen and played out from Johnny's perspective. These include Elizabeta's drug deal gone awry, and Ray Boccino's diamond operation (both of which were played from Niko's perspective in the main game). It is revealed that it was Johnny who kidnapped Roman Bellic and took him to the warehouse in Bohan where Niko eventually rescued him in the main game (Roman briefly hints this in a text message). It is also revealed that a seemingly random biker that Niko killed for Ray Boccino in the main game was actually Johnny's best friend, Jim Fitzgerald (in the mission No Way on the Subway). Also, Niko's killing of The Lost member Jason Michaels under orders from Mikhail Faustin is blamed on The Angels of Death by Billy Grey, who uses it as an excuse to re-ignite the gang war between them. The ending movie shows key points in the original story line, such as Niko and Roman finding their home and cab business destroyed, and Niko executing Vlad, with The Lost riding in the background. One of the missions also involves Tony Prince from The Ballad of Gay Tony, including the trading of Ray Bulgarin's diamonds, and provides Johnny's perspective to the first piece of plot to that expansion's storyline.

After witnessing the heist of the Bank of Liberty (carried out by Niko Bellic and Packie McReary), Luis Fernando Lopez enters the loft of his employer, owner of successful clubs Maisonette 9 and Hercules "Gay" Tony Prince. Tony is in serious financial trouble, having taken out massive loans from the Ancelotti crime family and Mori Kibbutz in order to keep his clubs running. He asks Luis to work for Mori Kibbutz ( who is actually Brucie's older brother) and Rocco Pelosi, an Ancelotti gangster, in order to satisfy his debts. Luis soon becomes acquainted with Yusuf Amir, an Emirati real estate developer who is interested in purchasing and franchising one of Tony's clubs, and Ray Bulgarin, a Russian crime lord he met in the clubs. Tony also plans to buy two million dollars worth of diamonds in order to sell them at a higher price, but members of The Lost Motorcycle Club intervene the trading and steal them, resulting in the death of Tony's current boyfriend Evan Moss. Luis manages to intercept a meeting to trade the diamonds and recovers them. Bulgarin soon reveals that the diamonds are his property, and believes that Luis and Tony colluded to steal them, marking them for death. Giovanni Ancelotti orders that the diamonds are to be used as a ransom payment for his daughter Gracie, who was kidnapped by Niko Bellic and Patrick McReary.
Rocco meets with Luis, and suggest that he should kill Tony in order to gain favor with Bulgarin, so that he will spare him. Though he contemplates doing so, Luis ultimately refuses and escapes Tony's club when Russian gangsters sent by Bulgarin assault it. Luis travels to Firefly Island to disrupt Russian drug operations and cuts off his main cash flow, and learns that Bulgarin is fleeing the city by plane within two hours. With the help of Yusef (who kills Bulgarin's henchmen attacking Luiz to stop him boarding Bulgarin's plane) with his Buzzard attack chopper, Luis manages to board the plane, and kills all of the henchmen on board. A vengeful Bulgarin emerges from the cockpit holding a grenade, threatening destruction to both of them if Luis kills him. Luis takes the risk and shoots Bulgarin. A dying Bulgarin drops the grenade causing the entire plane to explode. Luis parachutes out of the burning wreckage of the plane to safety, and heads to Meadows Park where Tony is waiting . Once there, they both tell Yusef, who has arrived at the scene, that they won't franchise the clubs since they prefer to keep it a "family business". In a small twist ending, the diamonds continually fought for are found by a homeless man in the trash.
Like The Lost and Damned, the story of the Ballad of the Gay Tony also intertwines with the main story of Grand Theft Auto IV, as well as The Lost and Damned. The first main part of the storyline that intertwines is the diamond deal. A cook aboard the ship Platypus finds the diamonds (as seen in the opening credits of Grand Theft Auto IV]], and the deal between Tony and the cook is disrupted by the Lost motorcycle club, also shown in the Lost and Damned. The second deal at the Libertonian with Isaac is also shown in both DLCs, as well as Grand Theft Auto IV. The trading of the diamonds in exchange for Gracie also is in both Grand Theft Auto IV and the game, as well as the aftermath of the diamonds, which had fallen into a dump truck, seemingly finding their way into a trash can, later found by the homeless man. Yusef Amir is mentioned briefly by Playboy X in Grand Theft Auto IV during the mission in which Niko and he attempt to take back one of his construction sites. Gay Tony is also briefly mentioned by "French" Tom Rivas, who talks about his bankruptcy. The big heist of the Bank of Liberty also features both Niko and Luis.
